A $1000.00 non-refundable deposit is due once surgery is scheduled and should be received to secure your surgery date. This deposit will be applied to your procedural fee. This amount is transferable should you need to reschedule for up to a years time. If you reschedule with our office more than three times, an additional $1000.00 non-refundable deposit will need to be collected prior to your new scheduled date. If you will need to cancel your surgery, we will need a 48-hour notice.
Please note: Price does not include pre-surgical testing for females (Tubal Reversals). Testing should be performed at location near you within 30 days of surgery. The Facility fee ($2736.00) of which $1824.00 is made payable directly to Broadwest Specialty Surgical Center (BSSC) and the anesthesiology fee is $912.00 is made payable to Bindal. This fee can be taken to Broadwest Specialty Surgical Center, LLC the day prior to surgery.
